HAUNTED HOLLER LIABILITY WAIVER, RELEASE OF CLAIMS & ASSUMPTION OF RISK

PLEASE READ CAREFULLY BEFORE SIGNING

Haunted Holler is a haunted attraction presented by Dodge City Campground. I understand that entering and participating in Haunted Holler involves inherent and other risks, both known and unknown, including circumstances intended to frighten, startle, surprise, confuse, or disorient participants.

ASSUMPTION OF RISK
I voluntarily assume the risks associated with entering or participating in Haunted Holler, including, but not limited to: darkness or limited visibility; uneven, wet, slippery, or obstructed walking surfaces; stairs, ramps, props, decorations, fog or theatrical effects; loud or sudden noises; flashing or strobe lighting; confined or crowded areas; sudden movements or scares; contact or collisions with objects or other persons; trips and falls; weather conditions; and the actions of other participants.

I understand these and other risks may result in property damage, emotional distress, illness, bodily injury, serious or permanent injury, or death.

HEALTH AND PERSONAL RESPONSIBILITY
I am responsible for determining whether I, or any minor for whom I am signing, can safely participate. I understand that Haunted Holler may not be appropriate for individuals with certain medical conditions, including heart conditions, seizure disorders, respiratory conditions, sensitivity to flashing lights, or other conditions that may be aggravated by fear, stress, darkness, fog, lights, sounds, or physical activity.

RELEASE AND WAIVER OF CLAIMS
To the fullest extent permitted by applicable law, I voluntarily release and waive claims against Dodge City Campground, Haunted Holler, their owners, operators, employees, volunteers, agents, representatives, and affiliated persons or entities for injuries, death, property damage, or other losses arising from the ordinary risks of participating in or attending Haunted Holler, including claims based on ordinary negligence, except to the extent such a release is prohibited by law.

RULES AND SAFETY
I agree to follow posted rules and instructions from Haunted Holler personnel. I understand that unsafe, disruptive, threatening, intoxicated, or inappropriate behavior may result in removal from the attraction without refund.

EMERGENCY MEDICAL CARE
If an emergency occurs, I authorize reasonable emergency assistance and understand that I am responsible for medical expenses incurred on my behalf or on behalf of a minor for whom I am legally authorized to sign.

MINORS
If I am signing for a participant under 18, I represent that I am the participant’s parent or legal guardian or otherwise have legal authority to execute this waiver on the minor’s behalf. I acknowledge and accept the risks described above on behalf of the minor to the extent permitted by applicable law.

ACKNOWLEDGMENT
I certify that I have read and understand this waiver, have had the opportunity to ask questions, and voluntarily agree to its terms. I understand that by signing the form below, I am giving up certain legal rights.

RULES AND SAFETY
I agree to follow all posted rules and instructions from Haunted Holler personnel. I understand that I may not intentionally touch, grab, hit, push, strike, threaten, chase, or otherwise make physical contact with any Haunted Holler actor, employee, volunteer, or other guest. I will not touch, damage, remove, or interfere with props, decorations, equipment, or sets.

I understand that actors may approach or startle me as part of the attraction, but this does not give me permission to intentionally make physical contact with them. Unsafe, aggressive, disruptive, threatening, intoxicated, or inappropriate behavior may result in immediate removal from the attraction without a refund and, when appropriate, may be reported to law enforcement.
